纳洛酮联合贯序无创通气治疗慢性阻塞性肺疾病合并呼吸衰竭患者临床疗效评价

摘    要:目的 探讨纳洛酮联合贯序无创通气治疗慢性阻塞性肺疾病合并呼吸衰竭患者临床疗效及对患者血气与肺功能的影响。方法 选取某医院收治的慢性阻塞性肺疾病合并呼吸衰竭患者94例作为观察对象,按随机表法分为观察组(47例)和对照组(47例)。对照组患者给予常规治疗,观察组行纳洛酮联合贯序无创通气治疗,对两组患者肺部功能情况、血气指标及临床疗效进行比较。结果 治疗后对照组的呼气高峰流量、第1秒用力呼气量及用力肺活量高于观察组,差异有统计学意义(P<0.05);治疗后观察组动脉血氧分压、血氧饱和度明显高于对照组,动脉血二氧化碳分压低于对照组,差异有统计学意义(P<0.05);观察组患者的总有效率为95.74%高于对照组82.98%,差异有统计学意义(P<0.05)。结论 纳洛酮联合贯序无创通气治疗慢性阻塞性肺疾病合并呼吸衰竭患者,临床疗效较好,利于促进患者血气指标及肺功能的恢复。

Abstract:Objective To investigate the clinical effect of naloxone combined with sequential noninvasive ventilation on patients with chronic obstructive pulmonary disease combined with respiratory failure and its effect on blood gas and pulmonary function. Methods A total of 94 patients with chronic obstructive pulmonary disease combined with respiratory failure were randomly divided into a trial group (n=47) and a control group (n=47). The patients in the control group were treated with the routine therapy, and the patients in the trial group were treated with naloxone combined with sequential noninvasive ventilation. The pulmonary function, blood gas index, and clinical efficacy were compared between two groups. Results After the treatment, the peak expiratory flow, forced expiratory volume in the first second, and the use of the vital capacity in the control group were higher than those in the trial group (P<0.05). After the treatment, the arterial partial pressure of oxygen and saturation of blood oxygen in the trial group were significantly higher than those in the control group, and the partial pressure of arterial carbon dioxide in the trial group was lower than that in the control group (P<0.05). The total effective rate in the trial group was 95.74%, which was higher than that in the control group (82.98%, P<0.05). Conclusion The clinical efficacy of naloxone combined with sequential noninvasive ventilation is better in the treatment of chronic obstructive pulmonary disease with respiratory failure, and this therapy improves the blood gas indexes and promotes pulmonary function recovery.
