重症心力衰竭急诊内科治疗临床分析

目的研究分析重症心力衰竭（refractory heart failure,RHF）急诊内科治疗的临床情况。方法 ２０１２年２月到２０１３年２月本院急诊内科共收治８６例RHF患者。随机分成观察组（４３例）和对照组（４３例）。对 照组实施常规RHF对症疗法，观察组在此基础上，给予联合美托洛尔以及厄贝沙坦氢氯噻嗪（irbesartan and hydrochlorothazide,IH）治疗。对比２组疗效以及治疗前后NYHA、LVEF及BNP等情况。结果观察组治疗后 的NYHA及BNP水平均显著低于治疗前，也低于对照组治疗后的水平；LVEF水平显著高于治疗前，以及对照组 治疗后的水平，差异有统计学意义（P＜０．０５）。观察组的总有效率为９５．３５％（４１／４３），显著高于对照组的８１．４０％ （３５／４３），差异有统计学意义（P＜０．０５）。结论常规疗法联合美托洛尔及IH治疗RHF疗效显著。

Clinical analysis of emergency internal medicine in treatment of refractory heart failure

Objective To study the clinical status of emergency internal medicine in the treatment of refractory heart failure（RHF）. Methods From February 2012 to February 2013,86 cases of emergency medicine patients were diagnosed as RHF and they were randomly divided into a trial group（43cases）and a control group（43cases）.The control group received the conventional RHF symptomatic therapy,and the trial group received metoprolol,and irbesartan and hydrochlorothiazide（IH）in addition to the conventional therapy. Results In the trial group,NYHA and BNP were significantly lower after the treatment;in the control group,the level of LVEF was significantly higher after the treatment（all P〈0.05）.The total effective rate of the trial group was 95.35%（41/43）,which was significantly higher than that of the control group（81.40%,35/43,P〈0.05）. Conclusions The conventional therapy combined with metoprolol and IH in the treatment of RHF has a significant effect.
