2007-2009年湖北省某市城区居民糖尿病疾病负担分析

目的估算十堰市城区糖尿病流行病学疾病负担,为优先配置卫生资源提供依据。方法采用2007-2009年十堰市城区居民死因监测资料,以伤残调整寿命年（DALY）为疾病负担测量单位分析糖尿病流行病学疾病负担。结果 2007-2009年,十堰市城区居民糖尿病死亡率和DALY损失随着时间增长而升高。2009年,由糖尿病导致的早死引起的生命损失（YLL）是2007年的2.06倍;总人群DALY为每千人2.90个寿命年,男性为每千人2.64个寿命年,女性为每千人3.11个寿命年,女性DALY损失高于男性,60岁以上人群为较大疾病负担人群,30~59岁年龄段DALY同样较高;70岁以上人群YLL率较高。结论十堰市城区糖尿病患者死亡导致的寿命损失逐年增加,应加强60岁以上人群糖尿病筛查和治疗,同时关注中青年人群。

Analysis on the burden of diabetes mellitus among residents in Shiyan city of Hubei province during 2007-2009

Objective To estimate the epidemiological burden of diabetes mellitus in Shiyan city and to provide the evidence for setting priorities in health resource. Methods According to the death survey data in Shiyan during 2007-2009,disability-adjusted life years（DALY） were used to analyze the epidemiological burden of diabetes mellitus. Results The mortality and DALY of diabetes mellitus in residents of Shiyan city enhanced with time during 2007-2009. The years of life lost（YLL） caused by diabetes mellitus in 2009 was 2.06 folds over that in 2007. The total DALY was 2.90 per 1 000,the male DALY was 2.64 per 1 000,the female DALY was 3.11 per 1 000. The DALY in females was higher than that in males. The DALY in older residents（〉60 years old） was higher,that was similar to the middle age residents（30-59 year old）. However,YLL of old residents（〉70 years old） was much higher. Conclusion YLL in patients with diabetes mellitus increases with time. The examination and treatment of diabetes mellitus should be conducted in old residents（〉60 years old）,also the middle age residents have to be paid attention to.
