急慢性肝病中血清AFU检测的意义

目的：为探讨血清α－Ｌ－岩藻糖苷酶（ＡＦＵ）在不同急慢性肝病中的变化及其意义；方法：应用微量分光光度法对２４５例正常人及各种急慢性肝病患者检测血清ＡＦＵ。结果：各组ＡＦＵ均高于正常对照组（Ｐ＜０．０１），其中原发性肝癌（ＰＨＣ）组最高（Ｐ＜０．０５或Ｐ＜０．０１）。急肝、中重型慢肝和活动性肝硬变各组血清ＡＦＵ均高于脂肪肝、ＨＢｓＡｇ携带者、轻型慢肝和静止性肝硬变（Ｐ＜０．０５或Ｐ＜０．０１）。肝癌以外各组ＡＦＵ与ＡＬＴ均呈正相关关系（Ｐ＜０．０５或Ｐ＜０．０１），而ＰＨＣ组二者无相关性。ＰＨＣ组与消化道肿瘤转移肝癌组血清ＡＦＵ、ＡＦＰ、ＣＥＡ阳性率差异明显。结论：提示血清ＡＦＵ在ＰＨＣ中敏感性较高，在其它急慢性肝病中也可升高，但似与肝损害程度有关，并不意味着恶性变。血清ＡＦＵ和ＣＥＡ在鉴别ＰＨＣ与消化道肿瘤转移肝癌方面有一定诊断价值。

CLINICAL SIGNIFICANCE OF SERUM AFU IN ACUTE AND CHRONIC LIVER DISEASE

Objective:To detect the change and significance of serum a-L-fucosidase (AFU) in patients with different acute and chronic liver disease.Methods:The AFU activity was microquantitatively measured by spectrophotometry in 245 cases of the patients with various acute and chronic hepatitis and healthy subjects.Results:The serum AFU activity in all liver disease groups was significantly higher than that in the healthy tontrol group (P<0.01).The AFU in the PHC group was the highest of the liver disease group (P<0.05 or P<0.01).There was positive relation between AFU and ALT in the liver disease groups except the PHC group.The serum AFU,AFP,CEA positive rates in the PHC group and the MLC group were 78.6%,64.3%,7.1% and 25%,0,75%,respectively.Conclusion:Sensitivty of AFU detection was higher in PHC.The positive AFU was also found in other liver diseases.The AFU was concerned with the degree of liver ingjury.AFU and CEA had some value of differentiating between PHC and MLC.
