TCT联合HPV在宫颈病变诊断中的价值

目的:探讨液基细胞学检查(TCT)和HPVDNA检测在子宫颈病变的诊断价值。方法:对某医院宫颈门诊1391例患者作TCT检测,所测阳性结果行HPV DNA检测以及阴道镜活检病理组织学检查,并以病理结果为金标准。对218例TCT阳性的患者,比较TCT、HPVDNA两种检测方法的敏感度。结果:TCT检测阳性者218例中,HPVDNA检测的敏感度为87.25%,TCT检测的敏感度为71.56%。两者相比差异有统计学意义(P0.05)。随着TCT及病理级别的升高,HPV阳性率也在升高。结论:对于宫颈癌及其癌前病变的筛查及早期诊断,TCT存在一定的假阳性和假阴性率,结合HPV检测可以提高宫颈异常细胞检出率,降低假阴性。

Value of TCT and HPV in the Diagnosis of Cervical Lesions

Objective：To Discuss the value of thinprep liquid-basedcytology test （TCT） and HPV DNA test in the diagnosis of cervical disease. Methods：A total of 1391 cases in Cervical outpatient were examined for TCT in the Third People＇s Hospital of Hefei, abnormal TCT were examined for HPV DNA test and for colposcope biopsy histology, And with pathological results for gold standard. In 218 cases of TCT positive patients, compare two kinds of detection methods sensitivity in TCT and HPV DNA test. Results：Among the 218 cases with abnormal TCT,The detection sensi- tivity by HPV DNA test was 87.25%, The detection sensitivity by TCT was 71.56%. Compared the difference was statis- tically significant.（P〈0.05）. With TCT and pathological level increases, HPV positive rate is on the rise. ConclusionzFor cervical cancer and precancerous lesion screening and early diagnosis, TCT has certain false positive and false negative rate. TCT Combined HPV DNA test can improve cervical abnormal cell detection rate and reduce false negative.
