多层螺旋CT对肺癌化疗患者治疗的预测价值

摘    要:目的探究多层螺旋CT灌注成像对肺癌化疗患者治疗的预测价值。方法随机选取我院92例肺癌患者进行短期放疗,对比肺癌患者化疗前后多层螺旋CT灌注成像结果。观察临床疗效、各灌注参数结果情况。结果随着化疗疗程的增加,肺癌患者化疗疗效逐渐提升,化疗4个周期临床疗效总有效率(44.57%),显著高于化疗1个周期(32.61%),P<0.05;随着化疗疗程增加,肺癌患者的TTP及TV不同时间点的变化呈逐渐改善趋势,P<0.05;化疗1、4个周期TTP和TV水平均显著低于化疗前,P<0.05;化疗4个周期的TTP和TV水平均显著低于化疗1个周期,P<0.05;化疗前,有效亚组BF值(96.33±17.87)ml/(min·100g)显著高于无效亚组,P<0.05;BV、PS及MTT值两亚组组间比较,差异不具有统计学意义,P>0.05;有效亚组化疗4个周期后BF值(63.89±9.65)ml/(min·100 g)、MTT值(6.01±0.81)s显著优于本组化疗前,P<0.05;无效亚组BV、BF、PS及MTT值与本组化疗前比较,差异不具有统计学意义,P>0.05;组间比较:化疗4个周期后,有效亚组BF、MTT值显著高于无效亚组,P<0.05。结论多层螺旋CT灌注成像能完全反应化疗前、后肺癌患者的血流灌注改变情况,能准确评价肺癌化疗疗效,值得推广使用。

Abstract:Objective To investigate the predictive value of multi-slice spiral CT in the treatment of lung cancer patients undergoing chemotherapy. Methods To select 92 cases of lung cancer patients,all patients received short-term radiotherapy. Also, the results of multi-slice spiral CT perfusion imaging in lung cancer patients before and after chemotherapy were compared. The clinical effect and the results of perfusion parameters were observed. Results With the increase of the course of chemotherapy, the curative effect of chemotherapy for lung cancer patients gradually also improved. The total effective rate in 4 cycles of chemotherapy was 44.57%, which was significantly higher than 32.61% in 1 cycle of chemotherapy, P <0.05;With the increase of chemotherapy course, TTP and TV in lung cancer patients gradually also improved,P<0.05;The levels of TTP and TV in 1,4 cycles of chemotherapy were significantly lower than those before chemotherapy, P <0.05;The levels of TTP and TV in 4 cycles of chemotherapy were significantly lower than those in 1 cycles of chemotherapy , P <0.05;Before chemotherapy, the BF value of effective subgroup (96.33±17.87)mL/(min·100g) was significantly higher than that of ineffective subgroup, P <0.05;There was no significant difference in BV, PS and MTT between the two subgroups, P >0.05;After 4 cycles of chemotherapy, BF ( 63.89± 9.65)mL/(min·100g) and MTT value (6.01±0.81) s in the effective subgroup were significantly better than those before chemotherapy, P <0.05;There was no significant difference in BV, BF, PS and MTT between the ineffective subgroup and the before chemotherapy in this subgrou,P>0.05;There was no significant difference in BV, BF, PS and MTT values between the ineffective subgroup before and after 4 cycles of chemotherapy, P >0.05. Conclusion Multi-slice spiral CT can completely reflect the changes of blood flow perfusion in patients with lung cancer patients before and after chemotherapy, and can accurately evaluate the efficacy of chemotherapy for lung cancer, which is worth using widely.
