摘    要:目的了解某基层医院近3年来铜绿假单胞菌的临床分布及耐药情况,指导临床合理用药。方法对从某医院2012年1月-2014年12月收集的各类临床标本分离出的铜绿假单胞菌进行鉴定和药敏试验,使用WHONET 5.4软件进行耐药分析。结果3年共分离出543株铜绿假单胞菌,占分离病原菌总数的15.2%,其中痰液标本301株,所占比率最高,达55.4%;其次是分泌物标本198株,占36.5%;543株铜绿假单胞菌主要分布在呼吸内科、ICU、烧伤科;药敏结果显示,543株铜绿假单胞菌除对多粘菌素B、亚胺培南、美诺培南、头孢哌酮/舒巴坦保持较好的敏感性外,对其他的抗菌药物的耐药率日趋增高。结论铜绿假单胞菌的分离率和对常用抗菌药物的耐药性日趋严重,应引起临床医务人员对其高度重视,规范临床合理用药,有效控制和减缓耐药菌的发生。

Abstract:Objective To investigate the clinical distribution and drug resistance of Pseudomonas aeruginosa in a hospital over the past three years in order to provide guidance for clinical rational use of drugs.Methods Different types of specimens were collected from inpatients between Jan 2012 and Dec 2014.Bacterial culture and identification were performed.Drug resistance was statistically analyzed using WHONET 5.4 software.Results A total of 543 strains of Pseudomonas aeruginosa were isolated from the clinical specimens over three years,accounting for 15.2% of all the isolates,including 301 (55.4%) isolates from sputum specimens that ranked first,followed by secretion samples from 198 strains,accounting for 36.5%.The 543 strains of Pseudomonas aeruginosa were mainly distributed in the Department of Respiratory Medicine,ICU and Department of Burn.Drug sensitivity results showed that the 543 strains of Pseudomonas aeruginosa not only remained sensitive to colistin B,imipenem,Miele imipenem,and cefoperazone / sulbactam,but kept increasing their resistance to other antibacterial drugs.Conclusion The isolation rate of Pseudomonas aeruginosa and drug resistance to commonly used antibiotics are increasing year by year,A good knowledge of the clinical distribution and drug resistance can better guide clinical rational use of drugs,thus effectively controlling and reducing the occurrence of drug resistant bacteria.
