脾动脉结扎加脾部分切除治疗脾破裂的探讨

目的 探讨选择性脾动脉结扎加部分脾切除在外伤脾破裂治疗中的临床应用.方法 对62例外伤脾破裂行脾动脉结扎加部分脾切除治疗的临床资料作回顾分析,总结病例选择、治疗方法、特点和注意事项.结果 除1例术后12 h内再出血,行脾切除外,61例一期手术均获得成功.术后免疫功能正常,保留的部分脾脏存活良好.结论 对Ⅱ～Ⅲ级脾破裂选择性行脾动脉结扎加脾部分切除治疗,是一种安全有效的治疗方法.

Research on rupture of spleen treated by splenic artery ligation combined with partial splenectomy

Objective To explore the clinieal application of selective splenic artery ligations combined with partial splenectomy in the treatment of rupture of spleen traumatic. Methods To review and analyze the clinical data of 62 cases of the rupture of spleen traumatic which had been treated by splenic artery ligation together with partial splenectomy, and smnmarize the choice of the cases, the ways of treatment to the cases, characteristics and considerations of the cases. Results All the cases treated in this way were successful except for one. After operation, the immune functions of those 61 successful cases were normal, and the retaining part of the spleen survived well. Conclusion For II-III grade line of splenic rupture, selective splenic artery ligation together with partial resection treatment is a safe and effective treatment.
