阴式子宫切除术与腹式子宫切除术的疗效观察

目的比较子宫切除术的两种不同术式对患者的影响。方法回顾性分析比较经腹（TAH）、经阴（TVH）子宫切除患者的手术时间、术中出血量、恢复肛门排气时间、镇痛药物使用率、术后病率及术后住院天数。结果阴式组较腹式组平均出血量少（P〈O．05），术后用镇痛药例数少（P〈0．01），术后排气时间早（P〈0．01），住院时间短（P〈0．01），术后病率少（P〈0．05），而对直肠膀胱功能的影响两组差异无统计学意义（P〉0．05）。结论TVH与TAH相比，手术时间短、创伤小，对腹腔脏器干扰小，术后疼痛轻、康复快、腹部不留瘢痕，更符合微创观念。

Clincal effect observation between vaginal hysterectomy and abdominal hysterectomy

Objective To evaluate the effects on patients of two methods with vaginal hysterectomy （TVH） and abdominal hysterectomy （TAH）. Methods A retrospective analysis and comparison Were made for abdominal hysterectomy and vaginal hysterectomy, in terms of the operation duration, blood lost during the procedure, postoperative analgesic, morbidity and hospitalization. Results Transvaginal hysterectomy was accomplished with less bleoding （ P 〈 0. 05 ）, less postoperative pain （P 〈 0. 01 ）, earlier anus exhaustion（ P 〈 0. 01 ）, and short postoperative stay at hospital（ P 〈 0.01 ）. No statistic difference was found between the groups in the effects on intestinum rectum and bladder function, operation duration. Conclusion TVH group is safer, more effective, less injury and pain, quicker recovery and no visible scar than TAH.
