单纯轻度尿检异常患者的临床和肾脏病理分析

目的：通过回顾性分析单纯轻度尿检异常患者临床与病理的特点，为单纯轻度尿检异常患者诊治提供参考。方法：选取本科室2009-2012年因单纯尿检异常而接受肾活检的患者77例，回顾性分析单纯轻度尿检异常患者肾脏病理类型的组成，根据肾脏病理将患者分为IgAN组、非IgAN组及基本正常组，对比不同病理类型组间各临床指标与病理的相关性。结果：77例患者中，肾脏病理基本正常11例，占14.3%；IgA肾病（IgAN）53例，占68.8%；非IgAN肾脏疾病13例，占16.9%。基本正常组的发病年龄、24 h尿蛋白定量（24 h upQ）均小于IgAN组和非IgAN组（P〈0.05）。结论：必须重视单纯尿检异常患者，特别是24 h upQ〉450 mg患者应当行肾活检，以期早期诊断和给予干预治疗。

Analysis of Clinical and Pathological of Patients with Mild Change of Urine

Objective： To study the relationship between clinical and pathological features,to provide reference of diagnosis and treatment for patients with mild change of urine.Method：77 patients with mild change of urine in our hospital from 2009 to 2012 were collected.According to the renal pathological,the patients were divided into the IgAN groups,the non IgAN group and the normal group,the pathological type and the relationship between renal pathological lesions and clinical indexes of the patients were analyzed.Result：Among the 77 patients,14.3%（11 cases）were normal renal pathology,68.8%（53 case）were IgA nephropathy（IgAN）,and 16.9%（13 cases）were another kidney disease. The age and 24 h upQ in the normal group were significantly smaller than the IgAN groups and the non IgAN group（P〈0.05）. Conclusion：We should be pay attention to the patients with mild change of urine,early renal biopsy and diagnosis are necessary when patients with 24 hupQ〉450 mg.
