综合锻炼方案对慢性阻塞性肺疾病稳定期老年患者的临床价值

目的评价慢性阻塞性肺疾病(COPD)稳定期老年患者应用综合锻炼方案的临床价值。方法对70例COPD稳定期老年患者,采用步行运动、腹式缩唇吹气法锻炼、吸气肌抗阻训练三位一体的综合锻炼方案,锻炼前、后分别采集患者生活质量评价多因素分级系统参数(BODE指数)、COPD评估测试评分(CAT问卷)、COPD急性加重频次、住院次数,并进行比较。采用症状自评量表(SCL-90)分析评价患者锻炼前、后心理问题检出情况。结果锻炼后,患者主观感受及客观症状均有不同程度改善,BODE指数3.2±1.2、CAT问卷评分(7.1±2.8)分,明显优于锻炼前(P<0.01);急性加重频次(0.7±0.2)次、住院次数(0.9±0.3)次,较锻炼前亦减少(P<0.05);心理问题总检出率、躯体化、强迫、人际关系、抑郁及焦虑状态检出率,均有不同程度的改善(P<0.05)。结论综合锻炼方案对COPD稳定期老年患者肺康复治疗,临床价值较高,锻炼过程安全,易于长期坚持,可有效改善临床症状体征,促进肺康复,同时可改善老年患者心理情绪状态,减少心理问题发生率,值得推广。

The clinical value of combined rehabilitation exercise in the treatment of elderly patients with stable COPD

Objective To evaluate the clinical value of combined rehabilitation exercise in the treatment of patients with sta- ble Chronic Obstructive Pulmonary Disease （COPD）. Methods Seventy elderly cases of stable COPD patients were selected and given combined rehabilitation exercise. The BODE index, CAT questionnaire score,the frequency of AECOPD,hospitalization frequency, and SCL-90 were calculated before and after the exercise respectively. The clinical value of the treatment was analyzed. Results Patients showed different degrees of improvement after the exercise. The BODE index and the CAT questionnaire score increased with statistical significance（P〈0.01）,while the frequency of AECOPD,the hospitalization in the combined exercise（P〈0.05）, and the occurrence rate of mental problems decreased （P〈0.01 or P〈0.05）. Conclusions Combined rehabilitation exercise is a safe and effective method to relieve the clinical symptoms and reduce the occurrence rate of the mental problems. This easily-conducted exercise is worth promoting.
