MRI增强扫描对胰腺癌和肿块型胰腺炎鉴别诊断的价值

目的探讨MRI动态增强扫描在胰腺癌和肿块型胰腺炎中的应用。方法选择某院胰腺癌患者22例,肿块性胰腺炎患者25以及正常胰腺20例,进行MRI三期动态扫描,计算各期强化率,绘制TIC曲线,并比较3组人群的差异。结果正常胰腺组、胰腺癌、肿块型胰腺炎3组的MRI动态增强TIC曲线具有较明显的差异。通过量化的数据分析,3组的研究对象在动脉早期强化率的总体差异具有统计学意义(F=1.78,P﹤0.05),两两比较后,胰腺癌组与其余两组差异有统计学意义(P﹤0.05),肿块型胰腺炎与正常胰腺组差异无统计学意义(P﹥0.05)。平衡期及延迟晚期,3组研究对象的强化率差异具有统计学意义(F=3.77及5.69,P﹤0.05),两两比较后的差异也具有统计学意义(P﹤0.05)。结论通过MRI多期增强扫描,TIC的差异能够有效地鉴别诊断胰腺癌和肿块型胰腺炎。

Value of MRI in diagnosis of pancreatic cancer and mass forming type pancreatitis

OBJECTIVE To explore the value of MRI in diagnosis of pancreatic cancer and mass forming type pancreatitis.METHODS A total of 22 patients with pancreatic cancer,25 patients with mass forming type pancreatitis and normal health people were collected.They were scanned by MRI dynamic scanning.The reinforcement rate was calculated,TIC curve was drawn,and the difference of the three groups were compared.RESULTS There was a significant difference in the reinforcement rate and TIC curve of the three groups.By quantized analysis,there was a significant difference in the early reinforcement rate of the three groups(F = 1.78,P﹤0.05),and there was a significance between pancreatic cancer and other two groups.However,no significant difference was found between mass forming type pancreatitis and normal health groups.There was a significant difference in the three groups of balance period and later period(F = 3.77 and 5.69,P﹤0.05).There was also a significant difference in each group.CONCLUSION By MRI multi-stage scanning,TIC could effectively identify the difference of pancreatic cancer and mass forming type pancreatitis.
