糖尿病合并呼吸道感染病原菌菌群分布与药物敏感性分析

摘    要:目的]探讨糖尿病患者合并呼吸道感染时病原菌的菌群分布与药物敏感性试验结果,为临床诊疗提供依据。方法]对92例临床确诊为糖尿病合并呼吸道感染的送检痰液标本进行微生物常规培养、鉴定(运用惠州阳光SS-1000全自动微生物鉴定仪)与药物敏感性试验(药敏试验采用K-B纸片法)。结果]在92份样品中共分离出微生物101株,其中革兰阴性菌属占54.46%,以铜绿假单胞菌及肺炎克雷伯氏菌多见;革兰阳性菌属33.66%,以葡萄球菌属为主;真菌占11.88%。药物敏感性试验显示革兰阳性菌对万古霉素保持较高的敏感率,达92.31%,而对青霉素的敏感率则仅为8.82%。革兰阴性菌对头孢他啶、阿米卡星、庆大霉素、环丙沙星等药物的药敏感率尚可,但对头孢唑林、氨苄西林的敏感率很低。结论]对患有糖尿病合并呼吸道感染的患者,有必要作微生物培养和药物敏感性试验,避免不合理用药。

Abstract:Objective]To analyze the bacteria distribution and the test results of drug sensitivity of patients diagnosed as diabetics combined with respiratory tract infection(RTI).Methods]92 sputum samples were collected from such patients and cultured;pathogenic bacteria were identified by SS-1000 Automatic microbiological analysis machine.Drug sensitivity tests were performed by K-B Disc diffusion method.Results]Of the 101 strains of pathogenic bacteria,54.46% were Gram-negative bacteria,dominated by Pseudomonas aeruginosa and Klebsiella Pneumoniae;36.66%(34/101)were Gram-positive,dominated by staphylococcus;11.88%(12/101)were fungi.Drug sensitivity results showed that 92.31% Gram-positive bacteria were sensitive to vancomycin,but only 8.82% of them were sensitive to penicillin.Gram-negative bacteria were sensitive to ceftazidime,amikacin,gentamicin,ciprofloxacin,but were not sensitive to cefazolin and ampicillin.Conclusion]To avoid antibiotics abuse on such patients,it is necessary to perform bacteria culture and drug sensitivity test.
