经腹、经阴道与腹腔镜全子宫切除术的临床对照分析

目的分析比较经腹、经阴道与腹腔镜全子宫切除3种不同方式的子宫切除术的临床效果。方法选择某院2010年1月～2011年11月行全子宫切除的72例患者作为观察对象,根据住院号随机将患者分为3组,即TAH组、TVH组、LTH组,每组各24例,对3组病例的手术时间、子宫质量、术中出血量、术后镇痛率、术后排气时间、术后住院时间、术后并发症进行比较。结果 LTH组病例手术时间明显长于TAH组、TVH组;TVH组和LTH组病例术后出血量、术后阵痛率、术后排气时间、术后住院时间明显较TAH组小或低;LTH组术后出血量较TVH组大,术后排气时间较TVH组短。差异均有统计学意义(P﹤0.05)。结论相比较而言,经阴道全子宫切除术更具有优势,多数情况下可作为首选,但在临床实践中,临床医生应综合考虑,在保证医疗安全的前提下,可合理地选择手术方式。

The comparative analysis of clinical abdominal,vaginal and laparoscopic hysterectomy

OBJECTIVE To analyze and compare the clinical effects of three different ways that abdominal,vaginal and laparoscopic hysterectomy.METHODS 72 patients in our hospital from January 2010 to November 2011 with total hysterectomy were chosen as the research subjects.According to hospital number,patients were randomly divided into three groups,namely group TAH,TVH group,LTH group,each group was with 24 cases.The operative time,uterine mass,blood loss,postoperative pain rate,postoperative discharge time,postoperative hospital stay,postoperative complications of cases in the three groups were compared.RESULTS The operative time in LTH group was significantly longer than the TAH group,TVH group.The postoperative bleeding,postoperative pain rate,postoperative discharge time,postoperative hospital stay of patients in TVH group and the LTH group were significantly less than the TAH group.Compared with TVH group,the amount of postoperative bleeding was more in LTH group,postoperative discharge time was longer in TVH group.Differences were statistically significant(P﹤0.05).CONCLUSION In comparison,vaginal hysterectomy has more advantages.It can be the first choice in most cases.But in clinical practice,clinicians should take composite consideration to ensure medical safety in the premise,it is reasonable to choose the surgical approach.
