老年肺结核合并糠尿病临床分析

目的观察肺结核合并糖尿病患者的不同血糖水平与肺结核病变程度的关系,以及不同降糖疗法控制血糖对抗结核疗效的影响。方法对烟台市北海医院2002年1月—2006年9月收治的60岁以上老年肺结核合并糖尿病123例进行综合疗法治疗。结果所有初治糖尿病肺结核80例,73例完成化疗方案,3例在治疗过程中因大咯血窒息死亡,4例因一般情况差,化疗失败。②所有复治的糖尿病肺结核43例病人,33例完成化疗方案。痰菌阴转、肺部病灶吸收稳定22例,随访0.5~1 a无复发;另6例虽完成化疗方案,但痰菌仍阳性,肺部病灶吸收不明显,化疗失败;2例在治疗过程中因大咯血窒息死亡,1例因一般情况差,全身衰竭死亡,1例不能耐受化疗。结论根据血糖水平,选择不同的降糖药物,是治疗肺结核合并糖尿病的良好方法。

Clinical Analysis of Senile Tuberculosis Combined with Diabetes

Objective]To observe the relationship between blood sugar level and tuberculosis changes among the patients of tuberculosis combined with diabetes and the influence of different blood sugar control therapies on the efficacy of anti-tuberculosis.Methods]Comprehensive therapies were conducted on 123 cases of senile(over 60 years old) tuberculosis combined with diabetes accepted from Jan.2002 to Sep.2006 in Beihai Hospital.Results] Among the 80 cases who received the first treatment,73 cases completed the chemotherapy,4 cases failed in the chemotherapy and 3 cases died from haemoptysis in the process of treatment;among the retreated 43 cases,33 cases completed the chemotherapy,6 cases failed in the chemotherapy,2 cases died from haemoptysis and 1 case died from systematic failure in the process of treatment and 1 case couldn't stand chemotherapy.Conclusion]To choose different blood sugar reducing drugs according to the blood sugar levels is a good method for treating tuberculosis combined diabetes.
