摘    要:目的借助循证医学的方法为重症急性胰腺炎(severe acute pancreatitis,SAP)患者确定治疗目标及治疗方案。方法在充分评估患者病情的基础上,提出临床问题并转换成便于检索的形式,检索Cochrane图书馆(2009年第4期)、SUM search(1990年1月—2009年1月)和PubM ed(1990年1月—2009年1月)等数据库,查找与治疗SAP的系统评价和随机对照试验,并对相关证据进行评价。根据证据结果及病人意愿为患者制定治疗方案。结果共检出与此不同问题相关的随机对照试验93篇,系统评价或M eta分析27篇,通过对检索结果进行分析并结合患者意愿,为患者制定了合理的治疗方案,经过治疗患者获得了良好的治疗效果。结论采用循证治疗的方法为SAP患者制定合理、安全的治疗方案,控制症状,预防并减少并发症,实现成本—疗效的优化。

Abstract:Objective] To make the therapeutic target and scheme for patient with severe acute pancreatitis(SAP) by means of evidence-based treatment.Methods]Based on an adequate assessment of the patient's condition,clinical problems were put forward and converted into the forms which were easy to search.The systematic reviews and randomized controlled trials(RCT) related to SAP treatment were searched in Cochrane library(Issue 4,2009),SUM search(January 1990 to January 2009) and PubMed(January 1999 to January 2009),and the evidences were evaluated.According to the evidence results and patient's wishes,the therapeutic scheme was made.Results]A total of 93 RCTs,27 systematic reviews and Meta analysis were identified.By analyzing the search results and patient's wishes,a rational therapeutic scheme was made.After treatment,the therapeutic efficacy was good.Conclusion]By means of evidence-based treatment,the rational and safe therapeutic scheme for SAP patients can control symptoms,prevent and reduce the complication,and optimize the cost-effectiveness.
