城市社区居家养老医疗服务模式探索

目的：探索建立城市社区居家养老医疗服务模式。方法：通过近两年的米市巷街道社区卫生服务中心的居家养老医疗服务工作的实践，探索建立居家养老医疗服务的可持续化模式。结果：米市巷街道的居家养老医疗工作实现了可持续化、标准化、网格化、信息化、中医特色化，有效提升了政府和社区卫生服务中心形象。结论：通过政府购买居家医疗服务，建立以政府为主导，社区卫生服务为依托，社会各界与志愿者广泛参与的居家养老医疗服务，是实现“老有所医”的一种可持续模式。

Urban community medical services of home--based elderly care

Objective To explore the establishment of urban community home care delivery model. Methods We established a sustainable model for medical services of home--based elderly care through nearly two years＇ practice in Mishixiang Community Health Service Center. Results The medical services of home-- based elderly care in Mishixiang Street Community had achieved a sustainable, standardized, grid, informational model characteristic of traditional Chi- nese medicine, which effectively enhanced the image of the government and community health service center. Conclu- sions The medical services of home--based elderly care is a sustainable mode to realize the goal of achieving elderly medical care, which is in the form of government＇s purchase of home health care service, built by taking the government as the leading factor,relying on the community health service and participation of all social ranks and volunteers.
