公立医院经济运行博弈分析和机制选择

公立医院经济运行是一个复杂适应系统,多方不完全信息下的动态博弈格局是公立医院在市场经济环境下资源优化配置不可或缺的经济过程,各方博弈的动态均衡是医院经济和谐运行的基础。在公立医院经济运行中,利益相关者的不当行为异化了公立医院经济运行主体的博弈行为,如政府的双重目标,使得医院注重医疗服务收入;医药市场无序竞争,医院进行成本转移;医疗保险效用不强,医患控费意识薄弱;患者盲目就医和监督不力,医师诱导医疗需求;医院考虑自身经济利益,默许过度医疗行为等。本文在分析公立医院经济运行的博弈特征和利益相关者的博弈行为的基础上,提出了促使公立医院经济活动达到相对均衡稳定的运行机制：利益关联机制、信息显示机制和激励约束机制。

Game Character and Mechanism Design of Public Hospital Economy Running

The economy running of public hospital is a complex adaptive system.In the market economy environment,it is an inevitable process for public hospitals＇ optimizing resources distribution to play a multi-player dynamic game with incomplete information.The economy of public hospital will run harmoniously while the game is dynamic equilibrium.In fact,the game strategies of the stakeholders of the economy running of public hospital have turned by their misbehavers.For example,the public hospitals pay attention to the income from medical services in corresponding to the double goals of government;there are out-of-order competitions in medicine market and the hospital transfer the cost to the patients;the function of medical insurance is not good and both the hospitals and the patients are not eager to save medical charge;the doctors induce medical demands with the patients in the low status;the hospital managers agree overspend medical resources by their needs.After analyzing the character of game and action of interest group,the paper puts forward the running mechanisms that promote the dynamic equilibrium of public hospital economy,such as interest association mechanism,information revelation mechanism,incentive-restraint mechanism,etc.
