重组酵母乙型肝炎疫苗免疫4年的持久性研究

摘    要:目的 观察研究国产重组酵母乙型肝炎疫苗的中、长期免疫效果,为控制乙型肝炎制定科学的免疫策略提供依据。方法 选择太原和阳泉市1995、1996、1997和1998年出生、筛查HBsAg阴性的儿童,用北京生物制品研究所生产的重组酵母乙型肝炎疫苗,严格按程序完成3针免疫,分别检测其免疫后1、2、3和4年的抗-HBs和GMT持久性水平。结果 免疫后1年的抗-HBs和达保护水平率均在92.31%,嗣后逐年下降。抗-HBs的下降速率不大,年平均下降1.44%,免疫后4年还保持在88.00%水平,各年下降水平比较差异无显著性;而达保护水平率的下降速率较快,每年平均达8.77%,免疫后4年的达保护水平率降至66.00%,免疫后1年与3、4年的保护水平比较,差异有显著性。结论 据本研究结果与疫苗本身的免疫学考虑,免疫后3~5年可不考虑加强问题,5年以后应考虑加强免疫。

Abstract:Objective To study on the mid and long- term effects of domestic recombinat hepatitis B (HB) vaccine and provide scientific basis for controlling HB.Methods The sera of children who were bom in 1995, 1996, 1997 and 1998 in Taiyuan and Yangquan Cities were collected and detected for HBsAg.The negative children were immunized with 3 doses of recombinat HB vaccine prepared with yeast cells by Beijing Institute of Biological Products, and the anti - HBs levels and GMTs of them were detected 1,2, 3 and 4 years after vaccination.Results Both anti - HBs positive and protective rates reached 92.31% lyear after vaccination, then decreased annually at average rates of 1.44% and 8.77% respectively. Four years after vaccination, anti - HBs postitive rate was still 88.00% , but the protective rate was only 66.00% .Conclusions Boosters should be considered 5 years after 3 doses primary immunization .
