北京市医耗联动综合改革对大型医院管理的影响分析

目的通过分析实施医耗联动综合改革后医院相关数据的变化情况,评价改革效果,进而对大型公立医院的内部管理和发展方向提出建议。方法以北京朝阳医院为例,采用自身同期对比分析方法,对实施医耗联动综合改革首月医院的医疗收入及患者的诊疗负担情况与改革前一年同期进行比较分析。结果改革后一个月与改革前同期相比,患者就医负担总体平稳,费用结构趋于合理;医院收入水平得以保障,收入来源和结构得到优化。结论大型公立医院应及时调整发展重心,明确急危重症和疑难复杂疾病诊疗的定位,改革与改善同步,调整完善医务人员的绩效分配与激励机制,转变医用耗材的管理模式,探索完善医疗项目价格体系,降低运营成本。

Analysis of the Impact of Joint Reform of Medical and Health Materials on the Management of Large Hospitals in Beijing City

Objective To evaluate the effect of the reform by analyzing the changes of hospital data after the joint reform of medical and health materials,and to put forward suggestions on the internal management and development direction of large public hospitals.Methods Through taking Beijing Chao-yang Hospital as an example,and by using self-comparative analysis method of the same period,doing a comparative analysis of the hospital’s medical income and patient’s burden in the first month of implementing the joint reform of medical and health materials with the same period of the year before the reform.Results One month after the reform,compared with the same period before the reform,the burden of patients’medical treatment is generally stable,and the cost structure tends to be reasonable;the level of hospital income is guaranteed,and the income source and structure are optimized.Conclusion Large public hospitals should timely adjust the development focus,clarify the location of diagnosis and treatment of critical and complex diseases,reform and improve synchronization,adjust and improve the performance distribution and incentive mechanism of medical staff,transform the management model of medical consumables,explore the improvement of the price system of medical projects,and reduce operating costs.
