基于奥马哈系统的延续护理对肝癌出院患者生命质量的影响

目的探讨基于奥马哈系统的延续护理对提高肝癌出院患者生命质量的影响。方法将某三级甲等专科医院80例肝癌术后出院患者随机分为观察组和对照组各40例,对照组采用常规延续护理教育,观察组采用基于奥马哈系统的延续护理教育。干预3个月后,定量测评两组患者在奥马哈问题系统中护理结局改善情况、自我护理能力和生命质量情况。结果护理结局改善方面:观察组在环境、社会心理、生理、健康相关行为4个领域问题的认知、行为和状况得分均优于对照组,差异有统计学意义(P<0.05);自我护理能力和生命质量方面:观察组ESCA各维度及QLQ-C30功能领域得分均明显上升,QLQ-C30症状领域得分明显下降,两组差异均有统计学意义(P<0.05)。结论基于奥马哈系统的延续护理能够使护理人员充分了解患者出院后的需求,针对护理问题提供健康教育和指导,可有效提高患者的护理结局和生命质量。

Effect of Continued Nursing Based on Omaha System on Quality of Life to Discharged Patients with Liver Cancer

Objective To explore the effect of continued nursing based on Omaha system on the quality of life to discharged patients with liver cancer.Methods A total of 80 in-patients with liver cancer who accepted surgical treatment were divided into observation group and control group with 40 cases for each group.The control group was administrated with routine transitional nursing education,while the observation group was additionally administrated with continued nursing theoretically based on Omaha system.It was quantitatively evaluated on the improvement of nursing outcomes,exercise of self-care agency and the quality of life score.Results The scores of problem in know-ledge-behavior-status,from four domains including environments,social psychology,physiology and health-related behaviors of patients in observation group were better than those in control group with significant differences 3 months after intervention.The patients in observation group evidently had higher scores in function domains of QLQ-C30 and dimensions of ESCA,but lower scores in status domains of QLQ-C30 after intervention,the differences were statistically significant(P<0.05).Conclusion The continued nursing based on Omaha system can enable nurses to understand the nursing needs of discharged patients and provide corresponding education and guidance according to problems,and can effectively improve the nursing outcome and quality of life of patients.
