摘    要:目的探讨老年代谢综合征(MS)合并急性脑血管病的临床特点。方法选择老年代谢综合征患者170倒,对其中经头颅CT或MRI证实脑卒中患者78例的临床及影像学资料进行分析,并和同期住院脑卒中非MS进行比较。结果MS组脑梗死72例(92.3%),脑出血6例,非MS组脑梗死66例(71.7%),脑出血26例,二者相比差异显著;MS组近期死亡率34.6%,非MS组9.7%;MS组合并肺部感染、呼吸衰竭、心力衰竭、肾功能衰竭及消化道出血机率较非MS组明显升高。结论老年代谢综合征合并急性脑血管病临床以脑梗死多见,较常出现肺部感染、呼吸衰竭等并发症,病情危重且死亡率高。

Abstract:Objective To explore clinical characteristics of senile patients with stroke complicated with metabolic syndrome (MS) and analyze their prognosis. Methods 78 senile stroke patients with metabolic syndrome were recruited, and 92 stroke patients without metabolic syndrome as controls. Results The prevalence of cerebral infarction was more significantly increasing in stroke patients in MS group than those in control group. The prognosis of patients in control group (9.7%) were better than the stroke patients in MS group (mortality = 34.6%). There was significant difference in severe complications, including pulmonary infection, respiratory failure, heart failure between MS group and control group (p 〈0.05). Conclusion The prevalence of cerebral infarction remarkably increased in senile stroke patients with metabolic syndrome. The mortality and incidence of complications in stroke with metabolic syndrome is higher, and usually associated with more severe stenosis.
