| 摘 要： | 目的探讨中性粒细胞体积、传导率和光散射(VCS)参数在急性细菌感染中的变化及临床意义。方法用Beckman-Coulter LH750全自动血细胞分析仪检测了45例急性细菌感染者(实验组)及27例健康体检者(对照组)外周血标本,统计分析了白细胞总数(WBC)、中性粒细胞(N)比率、中性粒细胞绝对值(ANC)以及中性粒细胞平均体积(MNV)、中性粒细胞体积分布宽度(NDW)、中性粒细胞平均电导(MNC)和中性粒细胞平均光散射(MNS)等VCS参数结果。结果实验组MNV和NDW较对照组明显升高,MNS较对照组明显降低(均P0.05),即使在白细胞总数不升高时;当MNV以142.2、NDW以19.42为临界值时诊断急性细菌感染的特异度分别为85.2%和96.3%,敏感度分别为80%和97.8%,后者明显高于WBC以10×109/L和N以0.85为临界值的敏感度(分别为57%、39%)。结论急性细菌感染时MNV与NDW明显升高,MNS明显降低是更客观和敏感的指标,对辅助诊断急性感染性疾病有一定的价值。
