434例急性脑卒中患者医院感染分析

目的探讨急性脑卒中患者医院感染的危险因素及其特点. 方法回顾分析434例急性脑卒中患者的病历资料. 结果本组分析434例急性脑卒中医院感染率为17.51%,与全院平均感染率5.99%相比,差异有非常显著性(P<0.01);伴意识障碍者感染率40.85%,远高于无意识障碍者12.95%,二者差异有非常显著性(P<0.01);有81.61%医院感染发生在住院初<2周;76例医院感染均使用了抗生素;行气管插管发生下呼吸道感染率92.00%,无气管插管者下呼吸道感染率8.07%,二者差异具有非常显著性(P<0.01). 结论脑卒中患者为医院感染的高危人群,其中病情危重、侵入操作及滥用抗生素是主要危险因素,入院初2周是医院感染的好发时段,应重点预防.

Hospital Infection Rate among 434 Cases with Acute Stroke

OBJECTIVE To study the risk factors and clinical features of hospital infection in 434 cases with acute stroke. METHODS The clinical data of 434 cases with acute stroke were retrospectively analyzed. RESULTS The hospital infection rate among 434 cases with acute stroke was 17.51%,and the hospital average infection rate in all was 5.99%,the former was significantly higher than the latter (P<0.01). The infectious rate of those with coma was 40.85%,and the infectious rate among those without coma was 12.95%,the former was significantly higher than the latter (P<0.01). There was 81.61% of hospital infection occurred in early two weeks,76 cases of them were used antibiotics. The hospital infection rate of lower respiratory tract of those with trachea intubation was 92.00%,and that of those without trachea was 8.07%,the former was significantly higher than the latter (P<0.01). CONCLUSIONS There is higher hospital infection rate among the acute stroke patients.The critical patients,invading operations and abusing antibiotics are principal factors. It is often to take place hospital infection within the first two weeks.We should prevent it in time.
