重型肝炎合并感染145例临床分析

目的 探讨医院感染对重型肝炎预后的影响，为加强医院感染的防治，降低病死率提供依据。方法 对1999年9月--2000年l2月收治的重型肝炎进行回顾性调查分析。结果 145例重型肝炎患者9l例发生医院感染感染发生率62．8％，医院感染率为64．8％，老年组和非老年组医院感染例次率分别为94．1％、60．9％(P＜0．05)；145例重型肝炎中病死率56．6％，感染组病死率74．7％，未感染组病死率25．9％(P＜0．01)。结论 感染是重型肝炎最多见的并发症之一，老年组感染率高，感染组病死率高；感染是肝衰竭最终导致多脏器功能衰竭的始动原因，合理应用抗生素，积极预防和治疗感染是提高重型肝炎存活率的重要环节。

Nosocomial Infections in Patients with Severe Viral Hepatitis: A Clinical Analysis of 145 Cases

OBJECTIVE To study the effect of nosocomial infection on the prognosis in patients with severe viral hepatitis(SVH) and to provide essential basis for preventing and treating nosocomial infections and decreasing the mortality of SVH. METHODS One hundred and forty five hospitalized cases of SVH were investigated and analyzed retrospectively during Sept. 1999 to Dec. 2000. RESULTS In 145 cases of SVH, there were 91 cases complicated by infection, the rate of infection was 62.8%, the rate of nosocomial infection was 64.8%, in the elder group was 94.1% and the younger group was 60.9%(P<0.05). The mortality of 145 patients with SVH was 56.6%, in the infective group was 74.7% and in the non infective group was 25.9%(P<0.01). CONCLUSIONS Infection is one of common complications in patients with SVH, there is a higher rate of infection in elder patients and a higher mortality in infective group. Infection is the original cause of hepatic failure and multiple organ dysfunction, adopting vigorous measure to prevent and treat infections by reasonable and effective use of antibiotics is pivotal for increasing the survival rate of the patients with SVH.
