| 摘 要： | 目的探讨宫颈细胞学检查联合高危型人乳头状瘤病毒（HPV）检测在宫颈癌、癌前病变筛查中的应用。方法对590名妇女采用传统巴氏涂片细胞学检查结合高危型HPV-DNA检测进行筛查，初筛异常和临床高度怀疑病变者进行阴道镜下多点活检或颈管内诊刮，以组织学病理结果为确诊标准。结果590例样本中，细胞学及HPV均阴性者无高度病变发生，细胞学阴性、HPV阳性者中大于等于宫颈上皮内瘤变（CIN）Ⅱ的检出率为10．1％，细胞学阳性及HPV阳性同时存在的病例中大于等于CINⅡ的检出率为39．4％，而仅细胞学阳性、HPV阴性者CINⅡ检出率为12．3％，无CINⅢ及以上病变发生。结论细胞学检查结合高危型HPV检测准确、快速、敏感，可用于大规模宫颈癌及癌前病变筛查，且阴性预测值可适当延长筛查间隔。
