脑梗死患者置鼻胃管对肺部感染影响的临床调查

目的了解不同程度吞咽功能障碍的脑梗死患者早期置鼻胃管对肺部感染的影响。方法采用回顾性调查的方法,对1556例急性脑梗死患者吞咽功能障碍的程度、早期置鼻胃管对肺部感染发病率的影响及病原菌的分布情况进行分析。结果 1556例脑梗死患者肺部感染率为10.60%,吞咽功能正常及可疑异常组的肺部感染率为1.97%;未置管组轻、中、重度吞咽功能障碍的感染率分别为8.05%、26.06%、51.52%,而置管组分别为7.32%、8.82%、9.09%;其中置管组、未置管组革兰阳性菌感染比例分别为17.86%、36.77%。结论对于中度以上吞咽功能障碍患者,及早予置鼻胃管鼻饲,能有效减少医院内获得性肺部感染的发生率;且置入鼻胃管患者的革兰阳性菌感染比例低于未置管患者。
