混苯及噪声暴露对工人脂肪肝患病率影响

摘    要:目的 了解混苯和噪声联合暴露对工人脂肪肝患病率的影响.方法 根据工作场所混苯与噪声暴露情况将634名工人分成4组,收集研究对象的基本情况,进行外周血谷丙转氨酶(ALT)、甘油三酯(TG)检测及肝脏B超检查.结果 混苯与噪声联合暴露组工人脂肪肝患病率为33.3%,ALT偏高率为23.8%,明显高于对照组的18.0%和14.0%(均P <0.05);logistic 回归分析表明,联合暴露组工人脂肪肝患病风险升高,OR=2.629(95%CI=1.412~4.898),外周血TG浓度和体质指数(BMI)升高者脂肪肝患病风险也增加,OR值分别为1.602(95%CI=1.320~1.946)和1.292(95%CI=1.187~1.406).结论 外周血TG、BMI及混苯与噪声联合暴露可增加接触工人患脂肪肝的风险.

Abstract:Objective To investigate the effect of co-exposure to benzene mixture and noise on the prevalence of fatty liver in workers.Methods Totally 634 workers were enrolled and divided into four groups according to their exposures to air benzene mixture and noise in workplace.Serum alamine aminotransferase(ALT),triglyceride(TG) measurement and liver B-ultrasonic examination were carried out.Results Compared with the control group,the workers with combined exposure of benzene mixture and noise had higher prevalences of fatty liver(33.3% vs. 18.0%) and higher ALT(23.8% vs. 14.0%)(both P <0.05).The results of logistic regression analyses showed that the workers with combined exposure to benzene mixture and noise had a higher risk of fatty liver(adjusted odds ratio[OR]=2.629,95% confidence interval[95%CI]:1.412-4.898) and the workers with increased TG or higher body mass index(BMI) also had a higher risk of fatty liver,with the OR of 1.602(95%CI=1.320-1.946) and OR of 1.292(95%CI=1.187-1.406),respectively.Conclusion The occupational co-exposure of benzene mixture and noise,increased serum TG and BMI may increase the risk of the fatty liver among workers.
