上海市社区高血压疾病管理比较研究

目的比较研究上海市社区高血压疾病细节管理、社会资源管理以及现行的社区高血压管理模式的运行效果。方法对闸北区3个社区430名高血压患者,开展为期1年的高血压社区综合干预。其中社会资源管理组172人,细节化管理组111人,对照组147人。结果干预后社会资源管理组高血压诊断标准的知晓率从62.2%上升到87.2%,每日食盐摄入量从4.6g减少到了4.0g,血压平均下降10.3/2.6mmHg。部分指标改善情况优于细节管理组与对照组。结论在社区高血压管理的过程中引入社会疾病管理资源可有效提高社区管理的效率,还可将社会疾病管理资源作为细节管理模式或者传统三级管理模式的补充,帮助提高现有模式的适用性。

Comparison of different disease management effect of hypertension in community of Shanghai

Objective To compare the effects of detail disease management,social disease management and the current hypertension management model in community. Methods A one-year comprehensive community hypertension intervention was carried on in 430 people of hypertension. All these people were assigned to an intervention group with social disease management resources （172 people）,a intervention group with detail management （111 people） and control group （147 people）. Results The awareness of hypertension of intervention group with social disease management resources was increased,the salt intake was decreased from 4.6g to 4.0g per day,and the blood pressure was decreased by 10.3/2.6 mmHg. Some of the indicators were better than the other groups. Conclusions The community hypertension intervention with social disease management resources can improve the efficiency of community disease management,and can be a complement of the detail disease mangement model or the traditional three-tire management model.
