3108例尿路感染病原菌及耐药性分析

摘    要:目的了解本院2017年尿路感染病原菌分布及耐药性分析,为临床合理用药提供依据。方法采用法国生物梅里埃公司全自动微生物分析系统VITEK 2 Compact及配套试剂对本院2017年1月1日-2017年12月31日的疑似尿路感染患者中段尿进行培养及进行药敏结果分析。结果共分离出3108株病原菌,检出最多的为大肠埃希菌875株(28.2%)、粪肠球菌271株(8.7%)、肺炎克雷伯菌233株(7.5%)、屎肠球菌208株(6.7%)。此外,分离真菌410株(13.2%)。女性患者居多(54.3%),年龄主要在81岁~90岁。耐药性分析发现,大肠埃希菌和肺炎克雷伯菌对亚胺培南耐药率分别为3.2%和35.8%;粪肠球菌和屎肠球菌对万古霉素耐药率分别为1.8%和0.6%,未检出对利奈唑胺耐药株。结论尿路感染病原菌中主要以大肠埃希菌和粪肠球菌为主,对各类药物有不同的耐药性,临床在合理用药方面应引起重视,切勿滥用抗生素加重多重耐药菌的流行。

Abstract:Objective To investigate the distribution and drug resistance of pathogenic bacteria of urinary tract infection in our hospital in 2017,and to provide basis for rational drug use.Methods The middle urine samples with urinary tract infections from January 1,2017 to December 31,2017 were conducted for culture and drug susceptibility analysis by VITEK 2 Compact,an automatic microbiological analysis system of French Bio Meriere Company.Results A total of 3108 pathogens were isolated.The most common pathogens were Escherichia coli in 875 cases(28.2%),Enterococcus faecalis in 271 cases(8.7%),Klebsiella pneumoniae in 233 cases(7.5%),and Enterococcus faecalis in 208 cases(6.7%).In addition,410 strains(13.2%)of fungi were isolated.Most of the patients were female(54.3%)and their age ranged from 81 to 90 years old.The resistance rates of Escherichia coli and Klebsiella pneumoniae to imipenem were 3.2%and 35.8%.That of Enterococcus faecalis and Enterococcus faecium to vancomycin were 1.8%and 0.6%.Neither of them was linezolid resistant.Conclusion Escherichia coli and Enterococcus faecalis are the main pathogens of urinary tract infections,and they have different drug resistance.We should pay attention to the rational use of antibiotics in clinic.
