不同亚型抗磷脂抗体、同型半胱氨酸与缺血性脑卒中的相关性研究

摘    要:目的探讨不同亚型抗磷脂抗体(a PLs)、同型半胱氨酸(Hcy)与缺血性脑卒中的关系。方法缺血性脑卒中患者233例,根据神经功能缺损程度评分分为重型组和轻型组;40例健康体检者为对照组。检测疾病组和对照组血清不同亚型抗心磷脂抗体(aCL)和抗β2糖蛋白1抗体(aβ2GPI)、Hcy、血脂、血糖。结果缺血性脑卒中患者血清IgG型aCL、IgA型aCL、IgG型aβ2GPI、IgM型aβ2GPI、甘油三酯、高密度脂蛋白、血糖水平与对照组比较,差异有统计学意义(P<0.05);神经功能缺损重型患者血清IgG型aCL、IgM型aCL、IgG型aβ2GPI、血糖水平明显高于轻型患者(P<0.05);IgG型aCL、IgG型aβ2GPI、甘油三酯、高密度脂蛋白、血糖与缺血性脑卒中发生有关(P<0.05)。结论IgG型aCL、IgM型aCL、IgG型aβ2GPI与缺血性脑卒中神经功能缺损程度有关,IgG型aCL、IgG型aβ2GPI可能是缺血性脑卒中的危险因素;Hcy与缺血性脑卒中神经功能缺损程度无明显相关关系,可能不是缺血性脑卒中的危险因素。

Abstract:Objective To investigate the relationship between different subtypes of antiphospholipid antibodies(a PLs),homocysteine(Hcy)and ischemic stroke.Methods A total of 233 patients with ischemic stroke were divided into severe group and mild group according to the degree of neurological deficit score;40 healthy subjects were selected as the control group.Serum anticardiolipin antibody(aCL)and anti-β2 glycoprotein 1 antibody(aβ2GPI),Hcy,blood lipids,and blood glucose were measured in case and control groups.Results The serum levels of IgG aCL,IgA aCL,IgGaβ2GPI,IgMaβ2GPI,triglyceride,high-density lipoprotein,and blood glucose in patients with ischemic stroke were significantly different from those in healthy controls(P<0.05).Serum IgG aCL,IgM aCL,IgGaβ2GPI,and blood glucose levels in patients with severe neurological deficits were significantly higher than those in patients with mild neurological deficits(P<0.05).IgG aCL,IgGaβ2GPI,triglycerides,high-density lipoprotein,blood glucose were associated with the occurrence of ischemic stroke(P<0.05).Conclusion IgG aCL,IgM aCL,and IgGaβ2GPI are related to the degree of neurological deficit in ischemic stroke.IgG aCL and IgGaβ2GPI may be risk factors for ischemic stroke.Hcy is not significantly associated with the degree of neurological deficit in ischemic stroke and may not be a risk factor for ischemic stroke.
