非脱垂子宫3种不同术式全切术比较分析

目的:探讨非脱垂子宫经腹子宫全切术(TAH)、阴式子宫全切术(TVH)和腹腔镜辅助下阴式子宫全切术(LAVH)3种术式的临床效果。方法:对TAH组、TVH组、LAVH组3组患者的术中、术后情况及治疗费用等方面进行比较。结果:LAVH组手术时间、术中平均出血量、肛门排气时间、术后住院天数明显少于TVH组、TAH组,但费用相对较高;在手术适应证方面,TVH组略小于TAH组和LAVH组,在安全性方面,TAH组则高于TVH组和LAVH组。结论:非脱垂子宫的3种全切术中,LAVH组术中和术后的指标优于TAH组和TVH组;但对于不同的患者,应综合考虑实际情况,采用相应的手术方式。

Comparison and analysis on three kinds of total hysterectomy in treatment of nonprolapsed uterus

Objective:To explore the clinical efficacies of total abdominal hysterectomy (TAH),transvaginal hysterectomy (TVH) and laparoscope assisted vaginal hysterectomy (LAVH) in treatment of non-prolapsed uterus.Methods:The intraoperative and postoperative situations and treatment fee in TAH group,TVH group and LAVH group were compared.Results:The operative time,intraoperative bleeding loss,anal exhaust time and postoperative hospitalization day in LAVH group were significantly shorter than those in TVH group and TAH group,but the treatment fee was higher; the range of surgical indications in TVH group was smaller than those in TAH group and LAVH group; the safety of operation in TAH group was higher than those in TVH group and LAVH group.Conclusion:Among the three kinds of total hysterectomy in treatment of non-prolapsed uterus,the intraoperative and postoperative indexes in LAVH group are superior to those in TAH group and TVH group; for different patients,appropriate methods should be adopted according to their different situations.
