C反应蛋白在小儿支原体肺炎中的测定及临床意义

目的:探讨血清C反应蛋白(CRP)检测在小儿肺炎支原体肺炎诊断中的临床意义。方法:分别采用免疫比浊及凝集法测定221例肺炎患儿血清CRP及血清支原体抗体IgM(MP-IgM)。同时测定61例正常对照组CRP。结果:132例支原体肺炎组(A组)CRP阳性者43例,阳性率32.58%,其中2例重症支原体肺炎者CRP大于80mg/L。89例细菌性肺炎组(B组)CRP阳性者83例,阳性率为93.26%,61例正常对照组(C组)CRP阳性者2例,阳性率3.28%。A、B组与C组相比存在显著性差异(P<0.01),A组与B组相比也有显著性差异(P<0.01)。结论:对于一般非重症肺炎,支原体肺炎者CRP稍高,但无细菌性肺炎增高明显,CRP可作为鉴别支原体肺炎或细菌性肺炎的重要辅助指标。

The measurement and clinical meaning on the CRP in infant with mycoplusmal pneumonia

Objective:To investigate the clinical significance of C-reactive protein(CRP)in infant with mycoplusmal pneumonia.Methods:Serum CRP and Mp-IgM levels were measured by nephelometric immunoassay and particle agglutination test in 221 infants with pneumonia.Results:Among the 132 cases with mycoplusmal pneumonia.〔A〕there were 43 with positive serum CRP,the positive rate for CRP was 32.58%,in which have 2 case with serious mycoplusmal pneumonia surpass 80 mg/L.Among 89 cases with bacteria pneumonia;〔B〕there were 83 with positive serum CRP,the positive serum rate was 93.26% Among 61cases with health children;〔C〕there were 2 with positive serum CRP,the positive rate 3.28%.The incidence of positive serum CRP in the A、B was significantly higher than in the C(P<0.01).Conclusion:The CRP in patients with mycoplusmal pneumonia isn't obvious high for general pneumonia.Detection of serum CRP levels may be of value in the differential diagnosis of pneumonia due to bacteria or mycoplasmal.CRP is obvious high no matter mycoplusmal or bactoria infection in severity pneumonia.
