| 摘 要： | 目的 分析团队管理群组模式在农村高血压患者管理中的应用效果,以期创建一个新型社区高血压管理模式.方法 以社区为基础进行社区干预项目设计,对高血压患者进行干预,比较干预前后各指标的变化情况,了解干预的效果.结果 干预12个月后,干预组患者在认知性症状管理技能应用、部分健康功能状况和血压控制方面较对照组好,差异有显著性（P＜0.05）.结论 团队管理群组模式能够帮助高血压患者改善自我管理行为、部分健康状况和降低血压水平.

| Abstract: | OBJECTIVE To analyze the effect of group team management model in the management of hypertensive in rural areas, to create a new community management of hypertension in order to. METHODS The communityintervention program for community based design, understand the effect of intervention by comparing the changes of each index, to understanel the effect of intervention. RESULTS After intervention for 12 months, patients in the intervention group, in theapplication of cognitive symptom management skills of health functionalstatus and blood pressure control compared to the control group. CONCLUSION Team management group mode can help hypertensive patients to improve the self-management behaviors, improve health status, reduce the level of blood pressure. |
