前置胎盘的流行病学特征及其期待疗法

目的 了解前置胎盘的流行病学特征及期待疗法对围产儿预后的影响。方法 对1992年1月-2002年12月收治的226例前置胎盘患者的病史采集及住院观察。结果 收治住院孕产妇总数为17610例，其中前置胎盘226例，其发生比例为1．28%；胎盘异常85例，占前置胎盘分娩总数的40．86％;流、引产次数与妊娠次数越多，前置胎盘发生率越高；通过期待疗法，尽量延长孕龄，降低了低出生体重儿的发生率。结论 预防前置胎盘的发生从根本上就应减少宫腔手术史；前置胎盘在无明显大出血及并发症情况下，能尽量期待至37周以上再予以分娩为佳.

Epidemiological Features of Placenta Praevia and Its Temporization

OBJECTIVE To explore the epidemiological features of placenta praevia and prognosi effects of temporization on perinatologic children.METHODS The data of 226 patients with placenta praevia who were hospitalzated in gynecological department of our hospital during Jan.1992 to Dec.2002 have been collected and analyzed.RESULTS During this period,there were 226 patients with placenta praevia among 17 610 gestational women who were treated in our obstetric faculty.Its incidence rate was 1.28 percent.85 patients with abnormal placenta were detected and was 40.86 percent of the total gestational women with placenta praevia.The study shows that the more times of abortion,induction or pregnancy one woman has,the higher prevalence rate of placenta praevia she may get;that the patients with placenta praevia were treated by temporization and elongated their pregnancy periods,which reduced the incidence of low birth weight children.CONCLUSIONS The basic methods to prevent placenta praevia should be decreasing uterus operation and to increase survival rate of perinatologic children should be enlonging pregnancy period to 37 weeks or more as long as the patients have not obvious hematorrhea and complication.
