我国慢性病直接经济负担研究

目的:通过对1993年和2004年《中国卫生统计年鉴》和第一次及第三次国家卫生服务调查相关数据的分析,测算5类慢性疾病的直接医疗费用。方法:直接医疗费用由出院人数、出院患者平均住院医疗费用、门诊人次数和门诊病人次均医疗费用求得。结果:2003年5类慢性疾病的直接医疗费用为1209.42亿元,其中,住院医疗费用588.37亿元,门诊医疗费用为621.05亿元:5类慢性疾病的直接医疗费用构成了国家医疗总费用的重要组成部分(占21.05%),其中,各病种(类)的费用负担在门诊和住院中表现不一,并且在城市主要以住院费用为主,在农村则以门诊费用为主;自1993年以来,其年平均增长速度为14.37%,超过了同期国内生产总值(GDP)、卫生总费用和医疗总费用的增长速度,其中,门诊病人次均医疗费用和出院者平均住院医疗费用的增长速度大大超过了同期的门诊人次数和出院人次数的增长速度,提示我国医疗行业存在垄断现象。

Economic Burden of Main Chronic Non-infectious Diseases in China

Object: On the base of data from China Health Statistical Yearbooks in 1993 and 2003 and the first and third National Health Services Survey, the direct medical expenses of the 5 main chronic non-infectious diseases are estimated. Method: The direct medical expenses are estimated by the combination of the average expenses per inpatient and the number of inpatients, and the average expenses per outpatient and the number of visits. Outcome: The total amount of direct medical expernses of the 5 main chronic diseases is RMB 120.942 billion including RMB58.837 billion inpatient expenses andRMB 62.105 billion outpatient expenses, which contribute to an important part of the total medical expenditures in China (21.05%) .Tere's difference in expenses distribution between the inpatient and outpatient across different chomic diseases. The total expenses of the inpatients are higher than those of the outpatients in urban areas, but the situation is reverse in rural areas.Since the year of 1993, the growth rate of the direct medical expenses of the five diseases has always kept 14.37% annually, which exceed the rate of GDP, total expenditure on health and total medical expenditures during the same time. The growth rates of the average medical expense per inpatients and outpatients are much faster than number of visits and inpatients, which implicated the monopolization in the medical industry of China.
