缩短平均住院日有效地提高医院的效率和效益

目的:通过对复旦大学医学院附属中山医院10年平均住院日情况的分析,为医院统筹管理,有效地缩短平均住院日,提高医院的效率和效益提供依据。方法:对平均住院日与病床利用情况指标进行相关性分析和住院病人的费用分析。结果:平均住院日与出院人数、病床周转率和病床使用率存在高度负相关,而与治愈率存在正相关(这与出院病人转归标准的重新界定及积极组织治愈病人出院有关)。并且P值显示平均住院日的长短对这4个指标的升降有着显著的影响。1996—2005年间,在核定床位增长了21.84%的情况下,平均住院日降低了45.02%。从“人均每日费用”、“与上年相比平均住院日下降天数”结合“出院人数”来分析,得出10年间由于平均住院日缩短而节省的住院费用达20284万元。结论:医院的统筹管理是缩短平均住院日的保证,缩短平均住院日应该在保证医疗质量的前提下力求“合理”、“可行”,缩短平均住院日只是手段,目的在于充分利用卫生资源,提高医疗质量和医院的综合效应,使医院朝着“质量”、“效率”和“效益”的方向发展。

Shortening Average Length of Stay Improve Medical Efficiency and Benefit in Hospital

Objective To shorten average length of stay and to improve medical efficiency and benefit by analyzing the average length of stay during ten years in our hospital. Methods Analyzing the data of the average length of stay,beds employed and medical costs. Result The average length of stay was in highly negative correlation with the number of discharge people,the beds velocity and the rate of beds employed. However,it had positive correlation with cure rate(this is related to the revised criteria of discharge). Also,the P-value showed the average length of stay had marked effect on the above indexes. During 1996-2005,along with the number of beds increased by 21.84%,the average length of stay dropped by 45.02%. In addition,the analysis revealed that the saved medical costs amounted to 202 840 000 RMB owing to the shortening of the average length of stay during the ten years. Conclusion Hospital managing as a whole is the guarantee of shortening of the average length of stay which should be on the basis of ensuring the medical quality. Shortening of the average length of stay is just a resort,the real aim is to fully utilize medical resources,to improve medical quality and medical efficiency and benefit.
