谷氨酰胺改善新辅助化疗引起的进展期胃癌肠黏膜通透性改变及毒性反应

摘    要:目的探讨口服谷氨酰胺对新辅助化疗(neoadjuvant chemotherapy,NACT)引起的进展期胃癌患者肠黏膜通透性、化疗相关毒性反应及化疗后病理反应的影响。方法入组江苏省肿瘤医院普外科2009年12月至2013年12月进展期胃癌(T3/T4,N+)患者45例,随机分为2组(A组:口服谷氨酰胺组,B组:常规对照组)。A组(25例)给予口服谷氨酰胺(10 g/次,3次/日,持续15天)+DOX方案新辅助化疗(多西他赛+奥沙利铂+卡培他滨),B组仅给予DOX方案新辅助化疗。分别于新辅助化疗前1天(NACT-1),后第14天(NACT+14),检测肠黏膜通透性,并记录化疗后不良反应情况。患者化疗2周期后手术,记录两组术后病理化疗反应情况。结果新辅助化疗后,A组恶心、呕吐(P=0.036),腹泻(P=0.034)发生率明显低于B组;肠黏膜通透性两组均较化疗前增加(均P0.001),但A组化疗后肠黏膜通透性增加水平明显低于B组(P=0.001)。两组新辅助化疗反应率未及明显差异(76%vs.70%,P=0.651)。结论口服谷氨酰胺能够有效改善进展期胃癌患者新辅助化疗后肠黏膜通透性增加,降低化疗相关消化道反应发生率,不增加新辅助化疗反应率。

Changes of intestinal permeability and toxicity effect in advanced gastric carcinoma induced by neoadju-vant chemotherapy through taking oral glutamine

Abstract:During treating wich NACT,compared with the ratio of L/Mof B group,the incidence of diarrhea,vomiting and nausea of A group was significantly lower.The L/Mratio of all patients increased after treating wich NACT,and the ratio of patients in B group was higher than that in A group.But no significant differences between two groups in pathologic response were observed in postoperative samples.Conclusions Oral glutamine could alle-viate NACT-induced increase in intestinal permeability and the toxicity,such as diarrhea,vomiting and nausea, but had no effect on the pathological response of gastric cancer.
