胸腔镜下肺叶切除术与常规开胸术治疗早期周围型肺癌的疗效对比

目的比较胸腔镜下肺叶切除术与常规开胸术治疗早期周围型肺癌的疗效。方法收集120例早期周围型肺癌患者,根据不同的手术方式分为试验组(80例)和对照组(40例)。试验组和对照组患者分别采用胸腔镜下肺叶切除术和常规开胸手术治疗。结果所有患者均痊愈,试验组患者的术中出血量、术后住院时间和术后第1天胸腔引流量均显著低于对照组(P<0.05)。试验组患者术后最大通气量(MVV)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、生理机能评分、生理职能评分、躯体疼痛评分和总体健康状况评分均显著高于对照组,差异有统计学意义(P<0.05)。结论与常规开胸术比较,胸腔镜下肺叶切除术具有术中创伤少、恢复快等特点,有利于患者肺功能恢复,提高患者术后生活质量。

The comparative efficacy thoracoscopic lobectomy and conventional thoracotomy for treating periph- eral lung cancer

Objective To compare the efficacy of thoracoscopic lobectomy and conventional thora- cotomy for treating peripheral lung cancer. Methods A total of 120 patients with early peripheral lung cancer were collected and divided into observation group （80 cases） and control group （40 cases） according to different surgeries. The patients of observation group and control group were treated with thoracoscopic 10- bectomy and thoracotomy surgery, respectively. Results All patients were cured. The blood loss, postop- erative hospital stay and chest drainage after the first day of the observation group were significantly less than those of control group （ P 〈 0.05 ）. The postoperative MVV, FVC and FEV1 values of physiology, physical function, bodily pain, general health scores of the observation group were significantly higher than those of control group （ P 〈 O. 05 ）. Conclusion Compared to conventional surgery, thoracoscopic surgery shows characteristics of less trauma and quicker recovery. It can benefit recovery of lung function and improve quality of life for lung cancer patients.
