传统与快速肠道准备方法对结直肠癌术后肠道微生态的影响

目的 探讨传统与快速肠道准备方法对结直肠癌术后肠道微生态的影响.方法 选取2008年2月至2016年3月进行结直肠癌手术的62例患者的临床资料进行回顾性分析,根据入院时间先后顺序分组,将2008年2月至2013年5月间进行传统肠道准备的患者30例作为对照组,2013年6月至2016年3月间进行快速肠道准备的患者32例作为观察组.对2组患者术中肠道清洁度、手术前后菌群计数及菌群失调情况、术后并发症进行观察与比较.结果 2组患者肠道清洁度比较,差异无统计学意义(P＞0.05).手术后2组患者双歧杆菌及乳酸杆菌数量均显著下降,大肠杆菌及粪肠球菌数量均显著升高.观察组变化幅度明显低于对照组,差异有统计学意义(P＜0.05).观察组手术后肠道菌群失调情况与术前差异无统计学意义,对照组肠道菌群失调情况明显加重,观察组与对照组比较有统计学差异(P＜0.05).观察组患者术后并发症发生率为9.38％,远低于对照组的40.00％,差异有统计学意义(P＜0.05).结论 对拟行结直肠癌手术的患者进行快速肠道准备安全有效,对肠道菌群影响小于传统肠道准备,值得临床推广.

The Influence of Intestinal Microecology of Normal and Rapid Bowel Preparation for Patients with Colorectal Cancer

Objective To explore the influence of intestinal microecology of normal bowl preparation (NBP) and rapid bowel preparation (RBP) for patients with colorectal cancer.Methods 62 patients with colorectal cancer were selected and divided into 2 groups according to the order of admission.30 patients underwent NBP were divided into the control group,and 32 patients underwent RBP were divided into the observation group.The intestinal cleanliness,intestinal floras counting and dysbacteriosis before and after treatment,complications after surgery of the 2 groups were compared.Results The intestinal cleanliness of the 2 groups had no statistically difference (P ＞ 0.05).The counts of bifidobacteria and lactobacilli were decreasing,e.coli and enterococcus faecalis increased after surgery,and the changing of the observation group was obviously lower than the control group,with statistically significance (P ＜ 0.05).The dysbacteriosis of the observation group after surgery had no big difference compared to before surgery,but the difference of the control group had statistically significance (P ＜ 0.05).The complications of the observation group was 9.38％,which was obviously lower than 40.00％ of the control group,with statistically significance (P ＜ 0.05).Conclusion RBP is a safe and effective methods for colorectal cancer patients who need to have surgery,which has less influence to the intestinal flora and it is worthy of clinical applications.
