全胸腔镜肺叶切除术对肥胖低肺功能肺癌患者心肺功能的影响

摘    要:目的 探讨全胸腔镜肺叶切除术对肥胖低肺功能肺癌患者术后心肺功能的影响.方法 将200例肥胖低肺功能肺癌患者根据随机数字法分为对照组(开胸肺叶切除术)和观察组(全胸腔镜肺叶切除术),每组各100例.观察和比较2组手术情况、术后心肺并发症,以及术后心肺功能.结果 2组术前2 d、术后7 d、术后6月体重、BMI比较,差异没有统计学意义,P>0.05.2组术后心肺并发症发生率,差异有统计学意义,P<0.05.2组术后7 d FVC、FEV1.0/FVC、MVV、FRC,差异有统计学意义,P<0.05;而术后6月观察组FVC、FEV1.0/FVC、MVV、FRC均有明显改善.结论 全胸腔镜肺叶切除术能够明显改善肥胖低肺功能肺癌患者术后心肺功能,并于术后6月基本恢复.

Effect of Completely Video-assisted Thoracoscopic Lobectomy on Cardiopulmonary Function of Obese Lung Cancer Patients with Lower Pulmonary Function
Abstract:Objective To study the effect of completely video-assisted thoracoscopic lobectomy on cardiopulmonary function of obese lung cancer patients with lower pulmonary function .Methods 200 obese patients with lower pulmonary function and lung cancer were randomized into the control group ( thoracotomy lobectomy ) and the observation group ( completely video-as-sisted thoracoscopic lobectomy ) according to the random number method ,100 patients per group .The operation ,postoperative car-diopulmonary complications ,and postoperative cardiopulmonary function of the 2 groups were observed and compared .Results There were no statistical difference in body weight and BMI on 2nd day pre-operation,7th day post-operation,6th month post-oper-ation between the 2 groups(P>0.05);there was statistical difference in the incidence of postoperative cardiopulmonary complica -tions between the 2 groups(P<0.05);there were statistical difference in the levels of FVC ,FEV1.0/FVC,MVV,FRC on 7th day post-operation between the 2 groups(P<0.05),and the levels of FVC,FEV1.0/FVC,MVV,FRC on 6 month post-operation were significantly improved .Conclusion Completely video-assisted thoracoscopic lobectomy can significantly improve the postopera-tive cardiopulmonary function of obese lung cancer patients with lower pulmonary function ,and the cardiopulmonary function can basically recover on 6th month post-operation.
