超声成像特征联合Mortalin蛋白、SMOC-2蛋白在子宫内膜癌诊断中的应用价值

摘    要:目的探讨超声成像特征联合寿命蛋白(Mortalin)、SPARC相关钙粘连分子2(SMOC-2)蛋白在子宫内膜癌诊断中的应用价值。方法选择96例子宫内膜癌患者,均能完成经阴道超声检查;术中收集癌组织和正常子宫内膜组织,采用免疫组化染色法测定Mortalin蛋白、SMOC-2蛋白表达,分析Mortalin蛋白、SMOC-2蛋白表达与超声成像特点对子宫内膜癌的诊断价值。结果子宫内膜癌患者超声成像主要见子宫内膜增厚和强回声,内膜与肌层分界模糊,受累肌层局部血流信号增多、血流供应丰富。经阴道彩超检查共确诊82例子宫内膜癌患者,诊断准确率85.42%,其中Ⅰb期检出准确率86.11%,Ⅱ期检出准确率87.50%,Ⅲ期检出准确率83.33%,Ⅳa期检出准确率80.00%。子宫内膜癌组织Mortalin、SMOC-2蛋白高表达率显著高于正常子宫内膜组织Mortalin、SMOC-2蛋白高表达率,差异有统计学意义(P<0.05)。Mortalin、SMOC-2蛋白联合诊断的AUC大于Mortalin、SMOC-2蛋白单独诊断的AUC(P<0.05),联合诊断的特异性和阳性预测值更高(P<0.05)。超声成像联合Mortalin、SMOC-2诊断子宫内膜癌的特异性和准确性均为96.88%(93/96)。结论 Mortalin、SMOC-2蛋白在子宫内膜癌中高表达,对诊断子宫内膜癌具有重要价值;超声成像联合Mortalin、SMOC-2蛋白可提高对子宫内膜癌的诊断价值。

Abstract:Objective To investigate the value of ultrasound imaging features combined with Mortalin protein and SPARC related modular calcium binding 2(SMOC-2) protein in the diagnosis of endometrial cancer.Methods 96 patients with endometrial cancer were enrolled, and all patients received transvaginal ultrasound examination.The expression of Mortalin and SMOC-2 protein in cancer tissue and normal endometrial tissue were assessed by immunohistochemical staining.Then the value of Mortalin protein and SMOC-2 protein expression and ultrasonic imaging characteristics in the diagnosis of endometrial cancer was evaluated.Results Ultrasound imaging of patients with endometrial cancer mainly showed endometrial thickening and enhanced echo, blurred boundary between the endometrium and muscle layer, and increased local blood flow signals and abundant blood supply in the affected muscle layer.A total of 82 cases of endometrial cancer were diagnosed by transvaginal ultrasound, with a diagnostic accuracy of 85.42%.The accuracy rate of transvaginal ultrasonography was 86.11% in detecting stage Ⅰb, 87.50% in detecting stage Ⅱ,83.33% in detecting stage Ⅲ and 80.00% in detecting stage Ⅳa.The high expression rate of Mortalin and SMOC-2 protein in endometrial cancer tissues was significantly higher than that in normal endometrial tissues, with statistical difference(P<0.05).Compared with single detection of Mortalin or SMOC-2,combined detection of the two indicators had significantly larger AUC,higher specificity and greater positive predictive value(all P<0.05).The specificity and accuracy of ultrasound imaging combined with Mortalin and SMOC-2 in the diagnosis of endometrial cancer were 96.88%(93/96).Conclusion Mortalin and SMOC-2 proteins are highly expressed in endometrial cancer tissues and are of great value in the clinical diagnosis of endometrial cancer, moreover, ultrasound imaging combined with Mortalin and SMOC-2 proteins present better diagnostic value.
