乳腺癌术前血常规参数与预后的关系

目的:分析中性粒细胞与淋巴细胞比值（NRL）、血小板与淋巴细胞比值（PLR）、红细胞体积分布宽度（RDW）和血小板平均体积（MPV）对乳腺癌患者预后的影响。方法:回顾性分析2005年1月-2010年7月31日，武汉大学中南医院收治的经手术治疗的228名乳腺癌患者临床资料。Log-rank单因素分析法评价各临床病理特征包括NRL、PLR、RDW和MPV对总生存时间的影响；采用Cox比例风险模型将有意义的变量进一步行多因素分析。结果:以NRL值2.485为界，将研究对象分为低NRL值组（NRL≤2.485）和高NRL组（NRL>2.485），两组的5年生存率具有明显统计学差异（P＜0.05）。多因素分析显示T分期、N分期、分子分型和NRL是影响乳腺癌患者预后的独立危险因素。结论:术前NRL水平可作为乳腺癌患者术后新的预后标志物。

Relationship between preoperative blood routine parameter and breast cancer prognosis

Objective:To investigate the relationship between preoperative blood neutrophil-to-lymphocyte ratio(NRL),preoperative blood platelet-to-lymphocyte ratio(PLR),red blood cell distribution width(RDW),mean platelet volume(MPV),and clinicopathological features，prognosis of patient with breast cancer.Methods:Clinicopathological data of 228 patients with breast cancer admitted to the Zhongnan Hospital of Wuhan University from January 2005 to July 2010 were analyzed retrospectively.Using the Logistic regression analysis to explore the influences of clinicopathological features,NRL,PLR,RDW and MPV on the overall survival.Using the Cox proportional hazards model to study meaningful variables line and factor analysis.Results:According to the preoperative NRL value,the patients were divided into NRL≤2.485 group and NRL>2.485 group.The 5-year survival rate between the two group had significant statistical significance (P＜0.05).Mutiple factors analysis showed that T stage,N stage,molecular subtype and NRL are independent risk factors affecting the prognosis of patients with breast cancer.Conclusion:Preoperative NRL level can be used as a new prognostic marker postoperatively in patients with breast cancer.
