氧化应激对多囊卵巢综合征患者体外受精-胚胎移植结局的影响

摘    要:目的探讨接受体外受精-胚胎移植(IVF-ET)的多囊卵巢综合征(PCOS)患者卵泡液中相关氧化应激指标的水平,并分析氧化应激状态对IVF结局的影响。方法募集进行IVF助孕的PCOS患者25例为PCOS组,因单纯输卵管因素行IVF治疗的患者23例为对照组,收集取卵日清亮卵泡液,检测多种氧化应激相关指标:脂质过氧化产物丙二醛(MDA)、晚期氧化蛋白产物(AOPP)、总抗氧化能力(TAC)、经典的酶类抗氧化物超氧化物歧化酶(SOD)、卵泡液抑制羟自由基(·OH)产生的能力、抗氧化辅酶Q(CoQ10)、还原型及氧化型谷胱甘肽(GSH、GSSG),并分析各项指标与IVF结局的关系。结果与对照组相比,PCOS组卵泡液GSH/GSSG较低,差异有统计学意义(P=0.036);PCOS组宫内妊娠率(35.29%)显著低于对照组(71.43%)(P=0.026);其他指标两组间无显著性差异(P0.05)。结论 PCOS患者卵泡液内存在GSH/GSSG比值异常的氧化应激状态,并可能影响其IVF-ET的临床结局。

Effect of oxidative stress on IVF outcome of patients with polycystic ovary syndrome

Abstract:Objective: To investigate the effect of oxidative stress on the pregnancy outcome of patients with polycystic ovary syndrome(PCOS)undergoing in vitro fertilization and embryo transfer(IVF-ET). Methods: The follicular fluid was collected on the egg retrieval day from 25 patients with PCOS (PCOS group)and 23 patients with simple tubal factor(control group)undergoing IVF-ET. The oxidative stress-related indicators, such as malondialdehyde (MDA), advanced oxidative protein products (AOPP), total antioxidant capacity(TAC), superoxide dismutase (SOD), the ability of follicular fluid to inhibit hydroxyl radical production, coenzyme Q10 ( CoQ10), reduced glutathione (GSH), oxidized glutathione (GSSG) were detected. Results: The ratio of GSH/GSSG in the patients with PCOS was significantly lower than that in the control patients(P=0. 036). The intrauterine pregnacy rate in PCOS group was significantly lower than that in control group (35.29% vs. 71.43%)(P=0. 026). There were no significant differences in other indicators between the two groups. Conclusions: The abnormal ratio between GSH and GSSG indicates that oxidative stress exists in the follicular fluid of the patients with PCOS,which can influence pregnancy IVF outcome.
