不育妇女卵泡液血管内皮生长因子水平与妊娠结局相关性

摘    要:目的 探讨卵泡液血管内皮生长因子 (VEGF)与妊娠结局的相关性。 方法 对 50例首次接受体外受精治疗的不育妇女 ,按其定量酶联免疫法测定卵泡液VEGF水平 ,编制频数分布表 ,分为 4组 ,记录患者卵母细胞质量、受精率、卵裂率及优质胚胎的百分比。 结果  (1 )卵泡液VEGF水平 450~ 50 9、51 0~ 70 9、760~ 1 0 0 9、1 0 1 0~ng/L时的受精率分别为 (49± 1 8) %、(71± 1 9) %、(52± 1 5) %、(3 4± 1 2 ) % ;卵裂率分别为 (68± 1 6) %、(84± 1 1 ) %、(71± 2 6) %、(71± 2 4) % ;优质胚胎比例分别为 (2 3±1 1 ) %、(74± 2 1 ) %、(50± 1 2 ) %、(2 2± 1 4 ) % ;妊娠率分别为 0 %、55%、45%、0 %。 (2 )相关分析结果 ,卵泡液VEGF水平越高卵细胞越成熟 ,但过高导致卵细胞退化 (r=0 .58,P<0 .0 5)。 结论 卵泡液VEGF水平过低或过高会导致卵母细胞质量、受精率、卵裂率及优质胚胎比例下降 ,导致临床妊娠率降低 ,卵泡液VEGF水平在 51 0~ 760ng/L ,妊娠结局最好

Abstract:Objective: To explore the relationship between follicular vascular endothelial growth factor (VEGF) level and the outcome of pregnancy in IVF cycles. Methods: In 50 infertile female patients under investigation, levels of VEGF in follicular fluid were measured by quantitative ELISA. The quality of oocyte, the rate of fertilization, cleavage and the embryo with good quality were recorded. Results: Too low or too high level of follicular VEGF will lead to poor fertile, cleavage rates and low percentage of perfect embryos (P<0.05). The oocyte maturity is correlated with follicular VEGF, but too high follicular VEGF level will lead to oocyte degeneration(r=0.58 ,P<0.05). Conclusion: Too low or too high follicular VEGF level will lead to poor pregnant rate. When it is within a proper range, the outcome of pregnancy is the best.
