人工关节置换治疗高龄股骨粗隆间骨折

目的] 观察人工髋关节置换在治疗高龄股骨粗隆问骨折病人的临床疗效。方法] 选择1998年10月～2004年9月采用双动人工股骨头置换术治疗高龄不稳定型股骨粗隆间骨折47例进行随访分析。结果] 经分析，所有病人在术后2～7d（平均3．4d）下地负重行走，术后3～6个月恢复到术前肢体功能，未发生重大并发症。结论] 对于严重骨质疏松且合并多系统疾病的高龄病例，采用人工髋关节置换治疗，可以尽早恢复肢体功能、早期下地负重、减少术后发症、提高生活质量，是内固定治疗很好的补充。

Treatment of unstable intertrochanteric fractures in elderly patients with cemented bipolar prosthetic replacement

Objective]To evaluated the clinical results of the bipolar prosthetic replacement for the treatment of unstable intertrochanteric fractures in elderly patients.Method]From 1998 to 2004, forty-seven cases with unstable intertrochanteric fractures were treated with bipolar prosthetic arthroplasty. All patients had some degree of osteoporosis and other function disordered. The clinical results were analyzed. Result]All patients had good clinical results over a period of 6~27 months follow-up and regained their pre-operative walking ability more often by the three-month review. Walking weight-bearing was begun 3.4 day (2~7d) post-operation. No serious complication occured.Conclusion]For elderly patients with unstable intertrochanteric fracture especially with extensive osteoporosis, cemented bipolar prosthetic arthroplasty may be the better choice than internal fixation in the advantage of earlier functional recovery and lower complications.
