摘    要:目的探讨3D打印个体化导向板在经皮椎体成形(PVP)术中的应用效果。方法纳入自2016-02—2017-02行PVP手术的椎体压缩骨折42例,21例采用3D打印个体化导向板辅助穿刺(导向板组),21例徒手穿刺(徒手组)。比较2组术中穿刺定位时间、透视次数及术后2 d的VAS评分。结果本组术中穿刺操作均顺利完成,术中及术后X线片证实骨水泥注入后弥散效果满意,无穿刺意外发生,无神经根及脊髓损伤。导向板组术中穿刺定位时间(4.57±1.17)min,透视次数(6.05±1.32)次;徒手组穿刺定位时间(15.24±1.70)min,透视次数(17.29±1.74)次;导向板组术中穿刺定位时间及透视次数明显少于徒手组,差异有统计学意义(P0.05)。导向板组术后2 d的VAS评分为(2.19±1.03)分,徒手组术后2 d的VAS评分为(2.14±1.28)分,2组差异无统计学意义(P0.05)。结论 3D打印个体化导向板辅助PVP治疗椎体压缩骨折的初步临床效果满意,可缩短手术时间、减少透视次数、增加手术安全性。

Abstract:Objective To investigate the application and effect of 3D printing individualized guide plate in percutaneous vertebroplasty.Methods From February 2016 to February 2017 PVP was performed on 42 cases of vertebral compression fractures,21 cases were treated with 3D print individualized guide plate auxiliary puncture (guide plate group),21 cases with free hand puncture(unarmed group).The puncture positioning time,fluoroscopy times and the VAS score 2 days after operation were compared between two groups.Results The puncture was successfully completed.The intraoperative and postoperative X-ray confirmed dispersion effect was satisfactory after bone cement injection.There were no puncture accidents,no nerve root and spinal cord injury.In the guide plate group,the puncture positioning time was (4.57±1.17)min,fluoroscopy times (6.05±1.32);In unarmed group,puncture positioning time (15.24±1.70)min,fluoroscopy times (17.29±1.74);The puncture positioning time in guide plate group was shorter than that in the unarmed group,the difference was statistically significant (P <0.05).The VAS score 2 days after operation in guide plate group was (2.19±1.03),and (2.14±1.28) in unarmed group,there were no statistical significant difference between two groups in VAS score 2 days after operation (P >0.05).Conclusion The preliminary clinical effect of 3D printing individualized guidance plate assisted PVP for vertebral compression fractures is satisfactory.It can shorten the operation time,reduce the fluoroscopy times and increase the safety of operation.
