微创经皮锁定加压钢板内固定治疗胫骨远端骨折

摘    要:目的探讨微创经皮锁定加压钢板内固定治疗胫骨远端骨折的效果。方法自2003年6月~2005年5月微创经皮锁定加压钢板内固定治疗胫骨远端骨折56例。按AO分类标准:A型20例,B型19例,C型17例。结果48例获得随访,平均11个月(8~21个月)。X线片显示骨折全部一期愈合,平均愈合时间为13周,均无感染、骨不连、钢板松动等并发症。按照Mazur踝关节功能评分:优38例,良6例,可4例,优良率为91.7%。结论微创经皮锁定加压钢板内固定治疗胫骨远端骨折符合生物力学固定(BO)原则,内固定牢靠,有利于骨折的愈合及软组织的修复。

Abstract:Objective To evaluate the preliminary outcome of surgical treatment of distal tibial facture with minimally invasive percutaneous locking compression plate(LCP) osteosynthesis.Methods From June 2003 to May 2005,56 patients with distal tibial fracture were treated with minimally invasive percutaneous locking compression plate(LCP) osteosynthesis.According to AO classification,twenty patients belonged to type A,nineteen type B,and seventeen type C.Results Forty-eight patients were followed up for 8 to 21 months,with an average time of 11 months.The X-ray pictures showed that one-stage union was achieved in the patients,with a mean healing time of 13 weeks.No complications such as infection,non-union,loosening of nails were found.The clinical outcomes were evaluated according to Mazur ankle joint function scoring.Thirty-eight were rated as excellent,six as good and four as fair,with the good-excellent rate being 91.7%.Conclusion Minimally invasive percutaneous locking compression plate osteosynthesis is a kind of biological osteosynthesis(BO),ensuring a good fixation in treatment of distal fracture with an advantage of accelerating bone and soft tissue healing.
