B超对溃疡病穿孔的诊断与动态监测

目的：确立超声检查在溃疡病穿孔中的诊断及中西医结合治疗中的动态监测声象图特征。方法：187例经临床、X线、胃镜、及手术证实的病人，行超声检查。结果：超声检查符合率93％(174／187)，其中超声检查腹腔内游离气体136例(73％)，假阴性48例(25％)，假阳性3例(2％)。结论：超声检查通过直接及间接声象图特征可及时准确诊断溃疡病急性穿孔，并能判断穿孔是否已闭合。

Diagnosis and Dynamic Monitoring Bi Zheming by Two Dimensional UItrasonography in Acute Perforation of Peptic Ulcer

Objective To establish the diagnostic method of ultrasonographic examination in acute perforation of peptic ulcer and the dynamic monoitoring ultrasonographic imaging feature in the treatment by integrated traditional Chinese and western medicine and to evaluate the application value Methods One hundres and eighty severe patients with acute perforated ulcer,diagnostically evidenced by clinical manifestations,X-ray,endoscopy and operforated were re-examined by ultrasonography and X-ray ,endoscopy and operation were re-examined by ultrasonography and X-ray to endure if the perforation has sealed or not.Results The accuracy rate of X-ray examination was 65%(121/187),false negativity rate was 35%.The accuracy rate of ultrasonographic examination was 93%(174/187).Among thesecases,there was free air in 136 patients(73%),in 48 patients it was false negative(25%),in 3 patients false positive(2%).Conclusion The ultrasonographic examination is a valuable diagnostic means in the acute perforation of peptic ulcer and is also useful in detecting the sealing of the perforation.It is the first-choice examination method and monitoring procedure.
