MRI动态增强联合DWI对乳腺良恶性病变的鉴别

摘    要:目的:探讨MRI动态增强时间-信号强度曲线(TIC)联合扩散加权成像(DWI)对乳腺良恶性病变的鉴别诊断价值。方法:收集经手术病理证实的43例(51个病灶)乳腺病变患者的MRT1WI、T2WI、DWI(b值=800s/mm2)和MRI动态增强扫描资料,对病灶信号强度、ADC值和增强幅度、TIC进行回顾性分析。结果:43例51个病灶中,TICI型曲线,良性19个,恶性1个;II型曲线,良性4个,恶性7个;III型曲线,良性2个,恶性12个;IV型曲线,6个病灶均为良性病变。病灶的ADC值>1.22×10-3mm2/s的30个(其中良性27个,恶性3个);ADC值≤1.22×10-3mm2/s的21个(其中恶性17个,良性4个)。结论:MRI动态增强TIC联合DWI对乳腺病变的诊断有较高的敏感性和特异性,TIC类型和ADC值对乳腺病变的良恶性鉴别具有重要价值。

Dynamic Contrast-Enhanced MRI Combined Diffusion-Weighted Image on Identification of Benign and Malignant Breast Lesions

Abstract:Objective To explore the diagnostic value of dynamic contrast-enhanced MRI time-signal intensity curve (TIC) combined diffusion-weighted imaging (DWI) in distinguishing the benign and malignant breast lesions.Methods Forty three cases (51 lesions) with pathologically confirmed breast lesions had MR T1WI,T2WI,DWI (b value=800 s/mm2) and dynamic contrast-enhanced MR scan.The signal intensity,apparent diffusion coefficient (ADC) values,enhanced extent and TIC of the lesions were retrospectively analyzed.Results The types of time-signal intensity curves in all 51 lesions(43 cases) included type I curve(19 benign and malignant one);II curve(4 benign,7 malignant);III curve(2 benign,malignant 12);IV curve(6 benign lesions).The ADC values of 30 lesions> 1.22×10-3 mm2/s (27 benign,3 malignant);ADC values of 21 lesions ≤ 1.22×10-3 mm2/s (17 malignant and 4 benign).Conclusion Dynamic contrast-enhanced MRI time-signal intensity curve combined DWI has high sensitivity and specificity in diagnosing breast lesions.The type of TIC and the value of ADC play important roles in differentiating benign and malignant breast lesions.
